I just had a little suggestion about the BT_PROFILE macro. Maybe it could be a good idea to change the syntax from BT_PROFILE("blah") to BT_PROFILE( blah ) and have the macro stringify it with #. The reason for this is if people (by coincidence, me ) would like to exchange the profiler macro with their own profiler. It's always possible to stringify the input, but it's not possible to unstringify it, so... Having the argument non-stringed leaves it more open.
