It's a really old question but I had the same problem and was able to solve it by increasing the number of solver iterations and the number of substeps. I had the same problem and it was because I was using concave shapes. You can use V-HACD (search it in the quickstart guide or use the test_vhacd executable if you have compiled Bullet).

Hi, I'm a beginner with Bullet and I'm trying to use it to detect collisions in an environment using industrial robots.
